Chat previews, issue reporting, and a smarter MCP setup

Fixes
- Chat now clearly surfaces MCP connector startup timeouts and Claude quota failures instead of failing silently.
- Fixed composer @-references being lost when switching AI profile on a draft or new chat.
- PDF attachments that can't be previewed now fall back to extracted text instead of failing.
- Fixed Codex runtime handling of unsupported chat file attachments and per-turn usage reporting.
Improvements
- Setting up a new MCP connector now offers to try it in a fresh chat; removed the redundant "try in chat" button from the connectors list.
- Skill installation is more reliable — release assets install straight from catalog config without depending on the GitHub API, and the skill catalog refreshes automatically after installing a directory skill.
- Presentation generation now draws generators from your installed skills, with the bento and ppt-master styles available as ordinary skills.
